The Valley Council Community Care Hub is an innovative collaboration designed to improve community health and quality of life for Valley residents.

We build relationships among health, social service, and access point organizations through a shared referral network, making it easier for community members to access the resources they need to stay safe and healthy.
The concept is simple yet powerful. When a community member is struggling with food insecurity, they are often facing other challenges like housing instability, mental health concerns, or difficulty paying for utilities. These problems are connected, and no single organization can solve them all. The Valley Council Community Care Hub helps providers work together to share referrals, reduce duplication, and collaborate so people get the support they need without navigating multiple systems on their own.
Created by the Valley Council for Health and Human Services, the Valley Council Community Care Hub is a community-wide initiative, bringing local organizations together to address these challenges and increase our collective impact. The Valley Council Community Care Hub is available at no cost to organizations or community members.

The Valley Council Community Care Hub uses Unite Us as its care coordination tool, allowing organizations to connect people directly to resources and track whether needs are met. This shared approach ensures no one falls through the cracks and services are provided in a respectful, coordinated way.

By working together through the Valley Council Community Care Hub, we can strengthen relationships, improve access to care, and build a strong support network for everyone in the Valley.
